Question 1: Locks and danger tags
The person who applied the danger tag is unavailable,
their tag or lock may only be removed by his trusted team member
Every person working on isolated equipment should fit their own lock and or danger tag.
Alternatively, another management approved system that achieves an equivalent level of safety may be used
There is no need that locks should be accompanied by
a corresponding tag to identify who has locked out the plant
Is not required to test that the isolation is effective by trying to reactivate the plant without
exposing the tester or others to risk so long as the person who did the isolation is competent

Question 2: Instruction, information, training and supervision
Training is not required if individual has got more
than 10 years of experience to do the isolation
All circuit breakers, switches and combined fuse switch units should be locked
off to secure the isolation where possible after the job has been done
If more than one person is working on the same de-energized electrical installation,
the supervisor should ensure his own personal lock is applied to the isolation point,
Appropriate instruction, information, training and supervision must be provided
to ensure that electrical equipment that has been de-energized to allow electrical work to be carried out is not inadvertently re-energized

Question 3: Locking off
Isolation involves using verbal warning or safety
signs as well as locks or other controls to secure the isolation.
Anyone can operate an isolator or knowingly use
equipment where the isolator has a control mechanism attached.
Isolation points should be fitted with control
mechanisms that prevent the electrical equipment from being inadvertently re-energized.
If the work is incomplete, due to change of shift, the last person removes their danger tag
or lock and replaces it only verbal warning is necessary to inform others

Question 4: Removal of Personal Locks
That the person who applied the danger tag is unavailable, their tag or lock may
only be removed in accordance with a management approved procedure
Fundamental principle is that the point of isolation should be under the control
of the team members who is carrying out the work on the isolated conductors.
When work is finally completed, only the supervisor
can remove danger tag and or lock.
Is not required to test that the isolation is effective by trying to reactivate the plant
without exposing the tester or others to risk so long as the person who did the isolation is competent

Question 5: Verification of Isolation
A check of system activation such as using a temperature gauge for electrical circuits
should be performed to ensure isolation. The equipment is now locked out
If there is a possibility of re-accumulation of stored energy, such as an increase in pressure
to a hazardous level, isolation of the equipment must be verified after repairs are completed
After ensuring that no workers can be injured, operate the push button or other normal controls
to verify that all energy sources have been disconnected and the equipment will not operate
Once all activities are completed, there is no need to get the permit to remove
the lock and tag and to restore the power
